Why enrol with us?

Enrolling with Devonport Health Centre means you’ll pay lower fees, receive publicly funded healthcare, and gain access to services through our PHO. It also allows us to keep your medical records up to date, making it easier to support your long-term health.

Enrolled patients benefit from more consistent, personalised care. You’re more likely to see the same GP or nurse, and having your health history in one place means we can offer advice and treatment that reflects the full picture.

Who can enrol?

To enrol, you must be eligible for publicly funded healthcare in New Zealand. Most citizens, permanent residents, and some work visa holders qualify. You can check your eligibility on the Health New Zealand website.

You can enrol children under 16 in your care by completing a separate form for each child. If a family member is 16 or older, they’ll need to enrol themselves unless you have Power of Attorney or are their authorised representative.

What are the benefits of practice-based enrolment?

Devonport Health Centre works on a practice-based enrolment model. This means when you enrol, you’re enrolling with the practice as a whole, rather than a single doctor.

This approach allows our team of GPs, nurses, and clinical staff to work together to support your care. It helps ensure your health information is shared appropriately across the practice and that you can still be supported if your usual GP is unavailable.

Practice-based enrolment offers:

Where possible, we aim for patients to see the same GP or nurse over time. Practice-based enrolment simply allows the wider clinical team to support your care when needed, helping ensure timely and reliable care.

What you’ll need

Before you start, make sure you have the following ready:

  • A valid form of ID (passport, birth certificate, or visa)

  • Address and contact information

  • A few minutes to complete the online form

Proof of your identity can be any one of the accepted documents listed in the eligibility checklist.
